As a convenience, we offer downloadable patient registration forms that you can print and fill out before your first appointment. Use the link below to locate these forms and fill them out prior to your first appointment with us.

If you are new to Walk-in Dermatology

If you are new to Walk-in Dermatology, you can request to have your previous medical records sent to our office prior to your appointment. Your Walk-in Dermatology physician will want to know your medical history including information regarding past illnesses, conditions and treatments. Listed below is a link to our patient registration forms, which includes the medical release form. Please print the form, fill it out and forward it to your prior physician’s office. This will ensure that your medical records are received prior to your first appointment with your Walk-in Dermatology physician.

How you can prepare for your appointment

Listed below are some helpful tips to prepare you for your appointment with us. If you have any questions, please call our office at (516) 621-1982.

  • Please arrive 15 minutes prior to your appointment to ensure all paperwork is filled out.
  • Please bring your insurance card, driver’s license or other picture ID, along with the required co-payment for your visit.
  • Complete all patient registration forms prior to your visit if you are a new patient.
  • Bring a list of your current medications, including any over-the-counter medications that you are taking.
  • Bring a list of allergies or any adverse reactions to medications that may have occurred in the past.

If you are unable to download and print the forms you need for a new patient visit, please arrive 20 minutes before your appointment to allow time to complete the forms.
